For the purpose of this Rider, the term 'Substance Abuse/Chemical Dependency Treatment Center' means a facility which provides a program for the treatment of substance abuse/chemical dependency pursuant to a written tt'eatment plan submitted by the Health Plan Psychiatric Primary Provider and approved in advance by CTHP and which facility is also: 1. Affiliated with a hospital under a contractual agreement with an established system fm' patient t'efet't'al, IJr, 2. Accredited as such a facility by the Joint Commission on Accreditation cf Hospitals) or) 3.
