<br />192 <br /> <br />ADMINISTRATIVE SERVICES AGREEMENT <br /> <br />CITY OF SAN MARCOS <br /> <br />This Agreement is entered into between above named City of San Marcos <br />(hereinafter referred to as the "Employer") and Boon-Chapman, Ltd., <br />(hereinafter referred to as the "Administrator") to be effective on <br />August 1, 1988. <br /> <br />WHEREAS, the Employer has adopted an employee health benefit plan known <br />as the Employee Group Life/Medical Dental Program which is set forth in <br />the Plan Document (hereinafter referred to as the Plan) attached to <br />this Agreement as Exhibit "A" and incorporated herein for all purposes, <br />for certain employees and their dependents (hereinafter referred to as <br />Covered Persons); and <br /> <br />WHEREAS, the Employer has requested Boon-Chapman, Ltd. to act as <br />Administrator, and Boon-Chapman, Ltd. has agreed to act as <br />Administrator and perform those services which are provided herein, in <br />accordance with the terms of this Agreement. <br /> <br />NOW THEREFORE, in consideration of the mutual promises and covenants <br />contained herein, it is hereby agreed as follows: <br /> <br />SECTION 1--ADMINISTRATIVE SERVICES <br /> <br />The Administrator shall provide the Employer with the following <br />services as required for the administration and operation of the Plan. <br /> <br />A. <br /> <br />Development, Communication, and Installation <br /> <br />1. <br /> <br />2. <br /> <br />3. <br /> <br />4. <br /> <br />5. <br /> <br />B. <br /> <br />Recommendations as to initial development and design of the <br />Plan and Plan Document and future revisions thereof. <br /> <br />Cost projections of benefits and administration. <br /> <br />Assistance <br />material. <br /> <br />in <br /> <br />preparation <br /> <br />of <br /> <br />employee <br /> <br />communications <br /> <br />Communication and enrollment of employees and dependents <br />through meetings provided and arranged by Employer. <br /> <br />Development and design of forms and procedures for porcessing <br />requests for benefits payment. <br /> <br />Reports and Records <br /> <br />1. <br /> <br />Preparation of such accounting reports as are needed in the <br />financial management and administrative control of the plan <br />such as: <br /> <br />a. projections of initial and renewal unit cost and total <br />cost; <br /> <br />b. <br /> <br />listings of benefits paid; <br />estimates of incurred but unpaid claim liabilities. <br /> <br />c. <br />
