-3- Administrator during the term of this Agreement. All files records, and reports shall be made available for inspection during normal business hours. Within ten (10) working days of completion of duties under this Agreement, all such files, records, and reports shall be turned over to the Employer on request and in exchange for its receipt. 4. Any protest of a benefit payment shall be brought to the immediate attention of the Employer. SECTION 2--FINANCIAL ARRANGEMENTS A. The Employer will establish an account with a bank of its choice in the name of the Plan, on or before effective date of the Plan. The Administrator will provide the Employer a listing of all Employees covered by the Plan on the effective date of the Plan. This listing will indicate dependent and all other coverages the Employer h~s. The Administrator will identify all plan costs for the Employer at this same time. B. c. The Employer will deposit monthly into the account established pursuant to section 2 (A), above, all employee contributions for all benefits into the Plan account. D. The Employer will remit monthly (by the lOth of the month) all life insurance premiums, optional employee premiums, and administrative charges to the Administrator. Upon receipt by the Administrator, the aforementioned premiums and charges will be remitted to the appropriate third party or insurance company, as applicable, in accordance with requirement of such third party or insurance company. E. The Administrator will issue benef it payments from an account established by the City of San Marcos. SECTION 3--INFORMATION PROVIDED BY EMPLOYER A. It is mutually understood that the effective performance of certain actuarial, claim, and underwriting services by the Administrator under this Agreement will require that the Employer furnish to the Administrator timely reports and information in a form and manner specified by the Administrator. Such reports and information shall include: 1. Upon presentation of a request for benefit payment, certification that the Covered Person is eligible for benefits under the Plan, identification of the types of benefits to which the Covered Person is entitled, date of eligibility and such other information as may be necessary for processing of claim payments; and Monthly statements indicating the number of employee and dependent units covered under the Plan. 2.
