<br />-3- <br /> <br />Administrator during the term of this Agreement. All files <br />records, and reports shall be made available for inspection <br />during normal business hours. Within ten (10) working days <br />of completion of duties under this Agreement, all such files, <br />records, and reports shall be turned over to the Employer on <br />request and in exchange for its receipt. <br /> <br />4. <br /> <br />Any protest of a benefit payment shall be brought to the <br />immediate attention of the Employer. <br /> <br />SECTION 2--FINANCIAL ARRANGEMENTS <br /> <br />A. <br /> <br />The Employer will establish an account with a bank of its choice <br />in the name of the Plan, on or before effective date of the Plan. <br /> <br />The Administrator will provide the Employer a listing of all <br />Employees covered by the Plan on the effective date of the Plan. <br />This listing will indicate dependent and all other coverages the <br />Employer h~s. The Administrator will identify all plan costs for <br />the Employer at this same time. <br /> <br />B. <br /> <br />c. <br /> <br />The Employer will deposit monthly into the account established <br />pursuant to section 2 (A), above, all employee contributions for <br />all benefits into the Plan account. <br /> <br />D. <br /> <br />The Employer will remit monthly (by the lOth of the month) all <br />life insurance premiums, optional employee premiums, and <br />administrative charges to the Administrator. Upon receipt by the <br />Administrator, the aforementioned premiums and charges will be <br />remitted to the appropriate third party or insurance company, as <br />applicable, in accordance with requirement of such third party or <br />insurance company. <br /> <br />E. <br /> <br />The Administrator will issue benef it payments from an account <br />established by the City of San Marcos. <br /> <br />SECTION 3--INFORMATION PROVIDED BY EMPLOYER <br /> <br />A. <br /> <br />It is mutually understood that the effective performance of <br />certain actuarial, claim, and underwriting services by the <br />Administrator under this Agreement will require that the Employer <br />furnish to the Administrator timely reports and information in a <br />form and manner specified by the Administrator. Such reports and <br />information shall include: <br /> <br />1. <br /> <br />Upon presentation of a request for benefit payment, <br />certification that the Covered Person is eligible for <br />benefits under the Plan, identification of the types of <br />benefits to which the Covered Person is entitled, date of <br />eligibility and such other information as may be necessary <br />for processing of claim payments; and <br /> <br />Monthly statements indicating the number of employee and <br />dependent units covered under the Plan. <br /> <br />2. <br />
