<br />GENERAL CONDITIONS OF AN <br />INDUSTRIAL WASTE PERMIT <br /> <br />1. <br /> <br />The Industrial Waste Permit application and Spill Prevention and <br />Control Plan must be on file and up to date. It is the <br />responsibility of the permitee to assure that accurate and <br />current information is reflected on both Industrial Waste Permit <br />application and the Spill Prevention and Control Plan. <br /> <br />2. <br /> <br />Any change in ownership of an industry may have an existing valid <br />permit for that industry transferred into his name by submitting <br />a new permit application containing sufficient information <br />regarding the discharge of pollutants including those referred to <br />in Section 307(a) of the Clean Water Act of 1977, and listed in <br />Appendix B to Title 40 Code of Federal Regulations Part 403. <br /> <br />3. <br /> <br />Violation of any of the specific or general conditions of this <br />Industrial Waste permit may result in the City ordering <br />disconnection of such person's service line from the City water <br />system and/or sanitary sewer and the same, at the approval of the <br />Director, shall only be reconnected at the expense of the owner <br />or occupant. <br /> <br />4. <br /> <br />In accepting this permit, the permitee certifies that all the <br />information recorded and on file with this permit is up to date <br />and correct. <br /> <br />-5- <br />
