<br />Nature of industrial operations, <br />Average rate of production, <br />Primary standard industrial classification, <br />Secondary standard industrial classification(s), <br />Average process wastewater flow <br />(gallons per day), <br />Maximum process wastewater flow <br />(gallons per day), <br />Average process plus non-process, plus sanitary flow <br />(gallons per day). <br />S.a. Identify the location for each sample point(s) and state if <br />the sample shall include process wastewater only, or <br />represent a combined (includes non-process wastewaters) <br />water system, <br />b. Identify the sample type <br />(grab or composite), <br />c. Identify which E.P.A. approved method <br />(Methods are listed in 40 CFR 136 and <br />Analysis Procedures for Screening of <br />for Priority Pollutants"), <br />d. A minimum of one sample analysis to report nature and <br />concentration of pollutants in the process wastewater <br />pretreatment discharge. <br />A statement, reviewed by a representative of the Industrial <br />Source, made by a qualified professional, (an individual <br />possessing academic or vocationally acquired credentials in <br />the theory and design of pollution abatement control methods <br />and procedures), indicating whether or not the discharge is <br />consistantly meeting the Pretreatment Standards, if <br />Standards are operations and maintenance, and/or <br />pretreatment facilities are needed.
