<br />7. <br /> <br />5. <br /> <br />Description of the Sampling and Analytical Program <br />a. Sample Point locations - Grab or Composite Type. <br />b. Wastestream compostion - Process only or combined. <br />c. Analytical Methods (as listed in 40 CFR 136 or other) <br />d. Analytical Report (see Appendix) <br />Discharge Quality Review Statement <br />A statement reviewed by the representative of the Industrial <br />Source made by a qualified professional whether or not the <br />discharge is consistantly meeting the pretreatment standards. If <br />Standards are not being met, then a statement that additional <br />operations and maintenance, and/or pretreatment facilities are <br />needed. If addditional operations, maintenance, or facilities <br />are needed, then a schedule of major events leading to compliance <br />shall be submitted. <br />Date, signature, and title of representative of the industrial <br />source submitting the report. <br /> <br />6. <br /> <br />Notes regarding Baseline and Quarterly Reports. <br /> <br />1. Existing sources who plan to introduce a new priority pollutant <br />or increase a monthly process wastewater average by more than 20%, <br />shall submit an arnmended report within 30 days of the change. A <br />minimum of two additional samples shall be taken within 30 days after <br />start up of the new discharge if a new pollutant has been added whose <br />process bath is followed by a process rinse. <br /> <br />2. New sources shall submit a Baseline Report at least 45 days prior <br />to start-up discharge. Samples shall be taken within 30 days after <br />start-up discharge. <br /> <br />3. Industrial Sources that use a sampling location that combines <br />process wastewater with any other non-process wastewater will have to <br />calculate Alternate Discharge Standards; based upon the ratio of the <br />monthly average to the daily average of the combined waste flows. <br /> <br />III. SELF-MONITORING AND REPORTING PROGRAM <br /> <br />Industrial Sources are required to periodically sample their discharge <br />and submit Quarterly Reports. <br /> <br />QUARTERLY PERIODS <br /> <br />1st Quarter <br />2nd Quarter <br />3rd Quarter <br />4th Quarter <br /> <br />January 1 - March 31 <br />April 1 - June 30 <br />July 1 - September 30 <br />October 1 - December 31 <br /> <br />Reports are due within 45 days after close of that quarter. <br /> <br />-32- <br />
