<br />NOTIFICATION OF CONTINUING SIGNIFICANT VIOLATION <br /> <br />The Director shall annually publish in the San Marcos Daily <br />Record newspaper a list of dischargers 'who continually have a <br />violation that: <br /> <br />Procedure: <br /> <br />1. <br />2. <br />3. <br /> <br />results in the exercise of emergency authority <br />remains uncorrected 45 days after notice of noncompliance is <br />given to the discharger <br />involves failure to accurately report information to the <br />Director as described in the applicable section of the Rules <br />and Procedures <br /> <br />At least once during the City's fiscal year the Director shall <br />publish a list of dischargers in the San Marcos jurisdic,tion who <br />continually have a significant violation of the Ordinance. The <br />Director shall notify each discharger on the list by certified mail <br />sixty (60) days prior to publication in the newspaper, that their name <br />will be published along with the facts about the violation including: <br /> <br />1. <br /> <br />Name of discharger <br />a) Corporate name (owner's name) <br />b) DBA (Doing Business As) <br />c) Names of key personnel involved in <br />Address of discharger <br />a) Corporate address <br />b) DBA address <br />c) Utility Service Address <br />Nature of the violation <br />Duration of violation <br />Remedies sought by the City <br />Enforcement steps taken by the City <br /> <br />discharging <br /> <br />2. <br /> <br />3. <br />4. <br />5. <br />6. <br /> <br />-59- <br />
