<br /> What conditions have beûn identified as cont~ibuting to the slum and <br /> blighted conditions? (Note: Froposed activities unde~ slum and <br /> blight must address the conditions contributing to the dete~ioration <br /> of the designated area.) <br /> The provision of housing assistance in an effort to eliminate blight <br /> & slum can not logically develop without the implementation of public <br /> facility improvements. The concerns of inadequate streets & drainage <br /> have contributed to some degree to the substandard conditions. Other <br /> components to the revitalization efforts would be the installation of <br /> curb & gutter, water & sewer lines. <br /> Applicants that qualify under the "Slum and Blight" national objective must <br /> still provide information on low/moderate income benefits -- Table 2 -- for <br /> pu~poses of scoring the application. If beneficiaries were dete~mined by <br /> su~vey, answe~ questions (a) through (c), page 15. If the applicant used <br /> census data, answer questions (d) and (e), page 15. <br /> (d) The applicant is providing a map which shows the boundaries of <br /> the E.D.'s that are impacted with blight & slum. <br /> (e) The enumeration districts that will qualify for housing rehabilitation <br /> assistance are 002 , 016 . <br /> -17- <br />
