accounts as provided by the City, the Contractor will bill the City on a per account basis for <br />each month. <br />B. Contractor's Personnel: The City will pay the Contractor for the provision of two employees <br />to operate the City's HHW Facility as stipulated in Subsection 1.3.2 at $17.78 per hour /per <br />employee. <br />C. City Facilities: The City will pay the Contractor for city facility pickup at a monthly rate of <br />$2,874.00. <br />D. Scrap Materials: The City's payment for the hauling of scrap materials will be deducted <br />from the total amount due to the City by the Contractor based upon the current market value <br />of the scrap materials collected from City facilities. If the container is hauled by truck and <br />trailer, the cost per haul is $30.00. If the pickup is a roll -off by a roll -off truck, the cost per <br />haul is $75.00 regardless of roll -off size. <br />E. E- Waste: The City will pay the Contractor $30.00 per pick -up of e -waste collected from the <br />City. <br />ARTICLE 4 <br />RECORDS AND REPORTS <br />Section 4.1 The Contractor will file the following records and reports with the City as directed: <br />4.1.1 Reports of all complaints received, investigations of these complaints, and actions taken by <br />the Contractor. <br />4.1.2 Reports of recycling information which includes volumes by category. <br />Section 4.2 The Contractor will retain all records and supporting documentation applicable to this <br />Contract, and not delivered to the City, for a period of three years except in the event that the <br />Contractor goes out of business, it will deliver to the City all of its records relating to this Contract <br />for retention by the City. <br />ARTICLE 5 <br />TERM; TERMINATION OF CONTRACT <br />Section 5.1 Contract Term <br />This Contract is a five (5) year contract effective from July 1, 2014 through June 30, 2019. <br />Section 5.2 Option to Extend <br />This contract may be extended for two (2) additional five (5) year terms, provided all terms and <br />conditions remain in full force and effect except for the contract period being extended.
