• 1- <br />DEPARTMENT OF STATE HEALTH SERVICES <br />,SPZ E OA <br />i ~ <br />w <br />x <br />H ~l <br />Amendment <br />To <br />The Department of State Health Services (DSHS) and CITY OF SAN MARCOS (Contractor) agree to amend <br />the Program Attachment # 001 (Program Attachment) to Contract # 2008-024654 (Contract) in <br />accordance with this Amendment No. OOIA : NSS -WIC CARD PARTICIPATION ,effective 05/01/2008 <br />The purpose of this Amendment is _ To increase participant rate extra funding increase by $30 680 re uired <br />due to unexpected EBT Implementation cost New rate 11 55 effective May 1 2008 SOW language revision <br />Therefore, DSHS and Contractor agree as follows: <br />SECTION II. SPECIAL PROVISIONS, first paragraph under section titled PARTICIPANTS SERVED PER MONTH <br />MAXIMUM REIMBURSEMENT, is replaced with the following: <br />PARTICIPANTS SERVED PER MONTH MAXIMUM REIMBURSEMENT: <br />During the term of the contract Attachment, Contractor shall earn administrative funds at the rate of <br />$8:96. 11.55 for each participant served as defined above. <br />SECTION II. SPECIAL PROVISIONS, fourth paragraph under section titled PARTICIPANTS SERVED PER MONTH <br />MAXIMUM REIMBURSEMENT, subparagraphs 1 and 2, are are revised and replaced with the following: <br />Contractor agrees that: <br />(1) Contractor shall identify and document separately not less than 19% of total administrative costs <br />as expenditures directly related to nutrition education and counseling with the exception of the <br />month of the Contractor's electronic benefits trap fer (CBTI implementation and the <br />following three months Nutrition education and counseling expenditures shall be supported by <br />documentation of participant attendance or non-attendance within the DSHS WIC Program. <br />(2) With the exception of the month of the Contractor's EBT implementation and the <br />following three months DSHS will reimburse Contractor for administrative expenses at a rate <br />not greater than 5.26 times the amount of properly documented expenditures for nutrition <br />education and counseling, but not more than is earned based on actual participation not to exceed <br />Contractor's assigned participant caseload, plus any incentive funds allocated to Contractor by <br />DSHS. <br />Page - 1 of 2 <br />
