-32- <br />(j) Floors. Floors shall be constructed of concrete <br />asphalt, tight wood, or other similar cleanable <br />material kept in good repair. Dirt or gravel, <br />when graded to drain, may be used as subflooring <br />whbn covered with clean, removable platforms <br />or Buckboards, or covered with wood chips, shavings <br />or other suitable materials effectively treated <br />to control dust. <br />(k) Walls and Ceilings of Food Preparation Areas. <br />(1) Ceilings shall be made of wood, canvas, or other <br />materials that protect the interior of the <br />establishment from the weather. Walls and <br />ceilings of food preparation areas shall be <br />constructed in a way that prevents the entrance <br />of insects. Doors to food preparation areas <br />shall be solid or screened and shall be self- <br />closing. Screening material used for walls, <br />doors, or windows shall be at least sixteen (16) <br />mesh to the inch. <br />(2) Counter-service openings shall not be larger <br />than is necessary for the particular operation <br />conducted. These openings shall be provided <br />with tight-fitting solid or screened doors or <br />windows or shall be provided with fans installed <br />and operated to restrict the entrance of flying <br />insects. Counter-service openings shall be <br />kept closed, except when in actual use. <br />Sec. 11-11. Compliance Procedures <br />(a) Permits, Licenses, or Certificates <br />(1) General. No person shall operate a food service <br />establishment who does not have a valid permit, <br />license, or certificate issued to him by the <br />regulatory authority. Only a person who complies <br />with the requirements of this ordinance shall be <br />entitled to receive or retain such a permit, <br />license, or certificate. Permits, licenses, or <br />certificates are not transferable. A valid <br />permit, license, or certificate shall be posted <br />in every food service establishment. <br />(2) Issuance of Permit, License, or Certificate. <br /> <br />1 <br />(A) Any person desiring to operate a food service <br />establishment shall make written application <br />for a permit, license, or certificate, on <br />forms provided by the regulatory authority. <br />Such application shall include the name and <br />address of each applicant, the location <br />and type of the proposed food service establish- <br />ment, and the signature of each applicant. <br />(B) Prior to approval of an application for a <br />permit, license, or certificate the regulatory <br />authority shall inspect the proposed food <br />service establishment to determine compliance <br />with the requirements of this ordinance. <br />(C) The regulatory authority shall issue a permit, <br />license, or certificate to the applicant if <br />its inspection reveals that the proposed food <br />service establishment complies with the require- <br />ments of this ordinance. <br />(D) Fees. <br />(i) The applicant for permits under this <br />division shall pay all fees to the tax
