Page 2 <br />Group A-7c <br />® Work or project activities typically involves major building or facility construction, site preparation; the installation, <br />replacement, or major modifications of energy system prototypes and infrastructure, access right-of-ways and roads; <br />utility, greenspace, and transportation infrastructure, vehicle test facilities; commercial buildings/properties, fuel <br />refinery/mixing facilities, factories/power plants; and other types of energy efficiency/conservation related systems, <br />structures, and facilities. To improve the energy efficiency of the city hall complex, the city proposes the <br />retrofitting the building as follows: the lighting, occupancy sensors with day-lighting controls, energy saving devices <br />on vending machines, HVAC system retrofits, EMS upgrades and retro-commissioning should be improved or installed <br />in the city hall complex. <br />List all other locations where proposed work or project would be performed by project's proposer and <br />subcontractors. The work will be accomplished at 630 E. Hopkins St. <br />10. Identify major project operation related materials and waste that would be used, consumed, and produced by this <br />project or activity. The retrofitting of the complex will produce some waste through demolition which will be <br />disposed of through the City's solid waste service. No hazardous materials will be used during construction. <br />11. Provide a brief description of the project location (physical location, surrounding area, adjacent structures). The City <br />Hall complex is in the center of town on its own city block. No adjacent structures. <br />12. Attach a site plan or topographic map of the project work area. <br />B.
