The Contractor and all subcontractors will be active and not debarred on the website, <br />�yy� ,:w.sam. 3t � and provide a copy of the certification to �e City before the entity perfos-ms work under this <br />contract. <br />The Contractor must ensure that awards are not made to any party which is debarred or suspended or is otherwise <br />excluded from or ineligible for participation hi Federal assistance programs under Executive Order 12549. <br />"Debarment and Suspension" and the LIAR_ Contractor will have each contractor and subcontractor complete a <br />contractor eligibility form in a format that is provided or approved by the Com. This form will provide the <br />information necessary to verify contractor eligibility. <br />E.
