A statement that a request for a hearing to determine whether or not the motor vehicle will be immobilized or impounded must be made to the municipal court in writing, without requirement of bond,within ten days after the date the notice was wed affixed to the vehicle; ed. A statement that failure to request a hearing before the expiration of the ten-day period, or failure to appear at a requested hearing will constitute a waiver of the opportunity to oppose the proposed impoundment or immobilization; and fe. A statement indicating that the registered owner of the vehicle, in addition to being responsible for all outstanding parking citations, will be responsible for all fees and costs incurred in immobilizing and impounding the vehicle, including storage fees.
