CONTRACT 214-331 <br /> E. Tires: The City will pay the Contractor the fees in Attachment A for its collection of <br /> abandoned tires after the first 450-3�5 tires collected per year <br /> F. Roll Off Containers (all sizes): thirty(30) day fee waived. Price includes delivery, hauling, <br /> disposal,fuel, environmental and rental fees. Additional fees will apply for the inclusion of <br /> TVs, CRTs, tires, and Freon bearing appliances. <br /> G. Nuisance Vehicles: The City will pay for the hauling and disposal of two nuisance vehicles <br /> per year. <br /> H. Mattress and Box springs (all sizes): The City will pay the contractor the fees in <br /> Attachment A for its collection of abandoned mattresses and box springs after the firstl,200 <br /> collected per year. <br /> ARTICLE 4 <br /> RECORDS AND REPORTS <br /> Section 4.1 The Contractor will file the following records and reports with the City as directed: <br /> 4.1.1 Reports of all complaints received, investigations of these complaints, and actions taken by <br /> the Contractor. <br /> 4.1.2 Reports of recycling information which includes volumes by category. <br /> Section 4.2 The Contractor will retain all records and supporting documentation applicable to this <br /> Contract, and not delivered to the City, for a period of three years except in the event that the <br /> Contractor goes out of business, it will deliver to the City all of its records relating to this Contract <br /> for retention by the City. <br /> ARTICLE 5 <br /> TERM; TERMINATION OF CONTRACT <br /> Section 5.1 Contract Term <br /> This Contract is a five (5) year contract effective from July 1, 2014 through June 30, 2019. <br /> Section 5.2 Option to Extend <br /> This contract may be extended for two (2) additional five (5) year terms, provided all terms and <br /> conditions remain in full force and effect except for the contract period being extended. This <br /> option, if exercised, is to be executed in the form of an Authorization of Change in Services,to be <br /> issued no sooner than one hundred and twenty days (120) prior to expiration of this contract, nor <br /> later than the final day of the contract period. This option to extend requires the mutual agreement <br /> of both parties. Refusal by either party to exercise this option to extend will cause this contract to <br /> expire on the original or mutually agreed upon date.
