Appendix 1 <br /> Subscription and Payment Terms <br /> 1. Covered Services-Subscription. The following services are covered as part of the subscription fee for each Member.All <br /> services provided are those medical services that the Primary Care Provider themselves are permitted to perform under the <br /> laws of the State of Texas and that are consistent with their training and experience in family medicine as a Primary <br /> Care Provider. All services included as part of the subscription for each Member include the equipment necessary to <br /> provide these services.Examples of services provided by Frontier include the following(complete list of services available <br /> upon request): <br /> a. EKG <br /> b. Laceration repair <br /> c. Skin tag/mole removal <br /> d. Skin biopsy <br /> e. Abscess incision and drainage <br /> f. Ingrown nail removal <br /> g. Foreign body removal <br /> h. Basic wound care <br /> i. Splinting/wrapping sprains, strains, or minor fractures <br /> j. Nebulizer treatment <br /> k. Office/Member visit <br /> 1. Urgent care visit <br /> in. Physicals: annual, sport,well-woman <br /> 2. Medications. Frontier provides a broad range of routine medications at no additional cost as part of the Subscription. These <br /> medications are dispensed by a pharmacy vendor and shipped directly to the Member,with shipping costs covered for up to <br /> six (6) shipments per year. Medications for urgent needs are also available through local pharmacy partners. If there is a <br /> change in the pharmacy vendor, both the Business and Members will be promptly notified. <br /> *Additional charges may apply and be billed directly to the plan. The Member is not required to utilize this service and may <br /> use the pharmacy of their choice. <br /> 3. Fee-For-Service Model Pricing* The services as shown below are available to all Members at the time of service and will <br /> be charged to the Member after the time the service is provided to the Member. Prices shown are approximate and may <br /> vary based on wholesale rates and/or market conditions. Frontier will provide a complete list of services and pricing upon <br /> request. <br /> a. Intravenous hydration *(Example Pricing$5) <br /> b. Albuterol inhalation*(Example Pricing$1) <br /> c. COVID-19 testing *(Example Pricing$12) <br /> d. Flu testing:A Virus and B Virus *(Example Pricing$15) <br /> e. Rapid strep test*(Example Pricing$2) <br /> *Additional charges may apply. These may vary based on wholesale rates and will be discussed with Member before <br /> purchase. <br /> 4. Price Disclosure.
