<br />LE'ITERS OF ENOORSEMENT <br /> <br />Each applicant is required to submit letters of support and endorsement from <br />each of the following applicable organizational units: <br /> <br />a) Appropriate officials or boards which govern the project <br />applicant <br /> <br />b) <br /> <br />City and/or county health departments <br /> <br />c) <br /> <br />City councils or county judges <br /> <br />d} Appropriate regional medic~l directors <br /> <br />e) <br /> <br />Area health systems agencies <br /> <br />These letters should be submitted with the application. Applicants may also <br />be asked to submit additional letters of endorsement from particular officials <br />or agencies. <br /> <br />12 <br />
