WOMEN, MEMORANDUM OF AGREEMENT BETWEEN INFANTS, AND CHILDREN (WIC) PROJECT #36 AND THE COMAL COUNTY HEALTH DEPARTMENT I. PURPOSE: The purpose of this agreement is to provide for a cooperative system of client referrals between the Women, Infants and Children (W.I.C.) Program and the Comal County Health Department. II. SCOPE: Both parties acknowledge that the majority of the people served by each program comes from the same tarket group, the low socio- ecomomic population. Both parties agree that a cooperative re- ferral agreement will enhance the services provided by each program. III. METHODOLOGY: W.I.C. Project #36 will refer all women, infant and children program participants to the Corna1 County Helath Department for primary health care whenever necessary. The Health Department will also have access to any medical-related information that W.I.C. has on these referrals. Said referrals will receive all consideration and services that other participants of the Health Department receive. It is further agreed that the Carnal County Health Department will allow the W.I.C. Project #36 to operate within their facilities as long as space and scheduling permit. W.I.C. will also receive re- ferrals made by the Health Department, and said referrals will re- ceive all consideration and services that other participants of the W.I.C. Program receive. This agreement becomes effective upon the signing of the agreement by the respective agency staff and until such time as deemed appropriate by Federal regulations. This agreement may be modified and amended in writing from time to time by mutual agreement and shall be implemented, interpreted and coordinated by the administrators of the two facilities. V.
