In accordance with applicable laws, certain employer based benefits such as the City's Internal <br />Revenue Code Section 125 Flexible Spending Account program will not be made available to the <br />Chamber's employees or their dependents. <br />3.2. The City will provide standard enrollment cards, benefit books and similar <br />information, materials and enrollment forms to facilitate participation and enrollment of the <br />Chamber's employees and their dependents. Benefit providers will be available to provide at <br />least one annual onsite educational and informational presentation to Chamber employees <br />regarding any of the benefits and products offered above. <br />3.3. The City will arrange for the processing of claims and provide the Chamber and <br />its employees with the necessary information to contact providers of services. <br />3.4. The City will deposit to the credit of the Chamber and its employees all premiums <br />collected and forwarded by the Chamber to the City. <br />IV. OBLIGATIONS OF THE CHAMBER <br />The Chamber will: <br />(a) pay for the total cost of coverage of its employees and their dependents <br />for the benefits provided through the Pool; <br />(b) educate employees on plan requirements including but not limited to strict <br />timelines for enrollment, termination, qualifying events and eligibility as defined in plan <br />documents. Changes submitted outside of these timelines will be returned. <br />(c) verify, as defined in the plan document, the eligibility of any persons who <br />request coverage under this Agreement; <br />(c) provide, in a timely manner, all information deemed necessary by the City <br />to administer the Pool and to obtain excess or stop loss insurance; and <br />(d) designate a coordinator in a management level position responsible for the <br />administration of the Chamber's obligations under this Agreement and participation in <br />the Pool to ensure compliance with the policies and procedures of the City, any third <br />party administrator and compliance with applicable law; <br />(e) disseminate to its employees all information provided by the City <br />regarding the Pool and any benefits as set forth in Article III; <br />(f) inform the City immediately of any changes in employee status and <br />eligibility for plan benefits such as newly hired eligible employees, changes in eligibility <br />such as hours worked or changes in employee or dependent status and the termination of <br />employment of any eligible employees; and <br />
