Section 13.16 Contractor's Notification of Change to Certain Contract Provisions. The <br />following changes may be made to this Contract without a written amendment or the Department's <br />prior approval: <br />a) contractor's contact person and contact information; <br />b) contact information for key personnel, as stated in Contractor's response to the Solicitation <br />Document, if any; <br />c) cumulative budget line item transfers that exceed 10% among direct cost categories, other <br />than the equipment category, of cost reimbursement contract Program Attachments of less <br />than $100,000, provided that the total budget amount is unchanged; <br />d) minor corrections or clarifications to the Contract language that in no way alter the scope of <br />work, objectives or performance measures; <br />e) a change in Contractor's share of the budget concerning non -DSHS funding other than <br />program income and match, regardless of the amount of the change, provided that in changing <br />the budget, Contractor is not supplanting DSHS funds; and <br />f) a change to remove community sites, independent school districts or schools, in substance <br />abuse Program Attachments in accordance with an Implementation Plan that must be <br />submitted along with the notification of the change to the contract manager assigned to the <br />Program Attachment. <br />Contractor within ten (10) calendar days shall notify in writing the contract manager assigned to the <br />Program Attachment of any change enumerated in this section. The notification may be by letter, fax <br />or email. Cumulative budget line item transfers of l0% or less among direct cost categories, other <br />than equipment, of cost reimbursement contracts of any amount do not require written amendment or <br />prior approval or notification. <br />Section 13.17 Contractor's Request for Revision of Certain Contract Provisions. A Contractor's <br />Revision Request is an alternative method for amending certain specified provisions of this Contract <br />that is initiated by Contractor, but must be approved by DSHS. The following amendments to this <br />Contract may be made through a Contractor's Revision Request, rather than through the amendment <br />process described in the Amendment section of this Article: <br />a) cumulative budget line item transfers among direct cost categories, other than the equipment <br />category, that exceed 10% of Program Attachments of $100,000 or more, provided that the <br />total budget amount is unchanged; <br />b) line item transfer to other categories of funds for direct payment to trainees for training <br />allowances; <br />c) change in clinic hours or location; <br />d) change in the equipment list substituting an item of equipment equivalent to an item of <br />equipment on the approved budget; <br />e) changes in the equipment category of a previously approved equipment budget (other than <br />acquisition of additional equipment, which requires an amendment to this Contract); and <br />f) changes specified in applicable OMB Circular cost principles as requiring prior approval, <br />regardless of dollar threshold (e.g., foreign travel expenses, overtime premiums, membership <br />fees); and <br />g) changes to add community sites, independent school districts or schools, in substance abuse <br />Program Attachments. <br />In order to request a revision of any of the enumerated provisions, Contractor shall obtain a Contract <br />Revision Request form from the DSHS website available at <br />General Provisions (Core Subrecipient) 2011 29 <br />
