<br />The City of San Marcos Facilities Service Contract <br /> <br />(7) a statement of any complaints received by the Company in relation to the <br />Contract Services and of how each complaint was addressed by the Company; <br />(8) a description of the maintenance activities performed by the Company <br />during the prior month and anticipated during the current month; <br />(9) a list of Facilities disposed of as surplus property pursuant to <br />Section 9.5; <br />(10) a list of machinery and equipment which was out of service during the <br />prior month, and a timetable for repair and replacement; <br />(11) an inventory of spare parts in stock and Consumables in storage at the <br />end of the prior month (quarterly if requested by the City); <br />(12) any adverse conditions which may reasonably be expected to anse <br />during the current month that may affect the ability of the Company to receive and <br />treat Influent, treat and dispose of Residuals, discharge Effluent or distribute Effluent <br />for beneficial re-use in accordance herewith; <br />(13) the results of any regulatory or insurance inspections conducted during <br />the prior month and all activities performed to comply with such inspections; <br />(14) information on any Utility outages occurring during the prior month; <br />(15) any failure to meet permit standards; <br />(16) a statement of any issues or violations related to regulatory compliance; <br />(17) a list and description of any accidents which occurred at the Facilities <br />during the prior month; <br />(18) a summary of the quantities of Residuals disposed of at the Facilities; <br />(19) a monthly discharge monitoring report; <br />(20) the results of any environmental, health or safety tests or monitoring <br />procedures conducted by any Governmental Body, during the prior month, and copies <br />of any reports or other submittals made to or received from any such Governmental <br />Body; <br /> <br />(21) any notices of violations of any Govemmental Approval received during <br />the prior month; <br />(22) a description of the repairs and Capital Modifications performed by the <br />Company during the prior month and anticipated during the current month; and <br />(23) any other data or information required to be fumished under the <br />Operation and Maintenance Manual. <br />(B) Annual Operations and Maintenance Reports. The Company shall <br />fumish the City, within 60 days after the end of each Contract Year, an annual summary of the <br />information contained in the monthly operations reports. The Company shall also perform <br />annual self-audits of Facilities operations and maintenance and provide the results to the City, <br /> <br />49 <br />
