<br />SAN MARCOS REVOLVING LOAN PROGRAM APPLICATION <br />CHECKLIST <br /> <br />THE FOLLOWING EXHIBITS MUST BE INCLUDED IN THE APPLICATION AND ALL EXHIBITS MUST BE <br />ORIGINALS. THE ORIGINAL AND A COPY MUST BE SUBMITTED TO THE CITY, IF THE EXHIBIT IS <br />NOT APPLICABLE, PLACE A SHEET OF PAPER UNDER THE APPROPRIATE LETTER AND STATE <br />N/A. THE COMPLETED APPLICATION SHOULD BE SUBMITTED IN A 3-RING BINDER WITH TABS <br />IDENTIFYING EACH EXHIBIT LISTED IN THIS CHECKLIST. <br /> <br />1. Complete the Notice of Intent Form. Sign and date the Business Certification Form and complete <br />the Credit Investigation Authorization Form. Label as Exhibit A. <br /> <br />2. Complete the Project Approval Information Form. Label as Exhibit B. <br /> <br />3. Provide a Comprehensive Business Plan addressing in detail the following information: <br />. Company history <br />. Product description <br />. Services <br />. Market <br />. Management <br />. Proposed job creation and/or retention (including a list of the job types to be created or retained) <br />. Description of Project and Financing Sources (including the interest rate, terms, and conditions <br />of the RLF loan request). Include any other sources of to be used, either equity or other loan <br />funds. <br /> <br />Label as Exhibit C. <br /> <br />4. Include a resume for each person with 20% or more ownership and also for the management team <br />for project. Label as Exhibit D. <br /> <br />5. Following the instructions provided, complete a Sources and Uses of Funds Form and provide <br />Budget Justifications for all project costs as outlined on the Sources and Uses Funds Form. Budget <br />Justifications must be provided to verify costs allocated to RLF and costs allocated to other funds. <br />Label as Exhibit E. <br /> <br />6. Using the instructions and samples provided, prepare all applicable financial commitment letters. If <br />the requested funds are to be in the form of a loan, include decline letters. Label as Exhibit F. <br /> <br />7. Complete the Request for Verification of Deposit Form for the business and for all other funds as <br />outlined on the Sources and Uses of Funds Form. Label as Exhibit G. <br /> <br />8. Complete the Employment Projections Form using the instructions provided. Label as Exhibit H. <br /> <br />9. Using the instructions and samples provided, prepare all relevant employment commitment letters <br />(created/retained). Label as Exhibit I. <br /> <br />10. Submit the completed Family Income/Size Certification for each retained job. For jobs to be <br />created, the Family Income/Size Certification will need to be completed on an on-going basis and <br />submitted to the City of San Marcos. Label as Exhibit J. <br /> <br />11. Prepare a Resolution stating who is authorized to incur indebtedness and enter into contracts. <br />Include this with the filed Assumed Name Certificate, Partnership Agreement, Corporate Charter <br />(Articles of Incorporation) and the Incorporating By-Laws (whichever applicable).
