<br />3. Additional Documentation. If requested by CAPCO, the SUBCONTRACTOR agrees to <br />provide to CAPCO the additional expense records and documentation materials, as <br />listed in Section (b) 2 of this Article and appropriate for the expense, for the time period <br />requested by CAPCO, except that the SUBCONTRACTOR will not be asked to submit <br />records that have already been provided to CAPCO with a Financial Status Report. <br />CAPCO will provide reasonable time for the SUBCONTRACTOR to comply with a <br />request for additional records. If CAPCO requests to review additional records to be <br />provided by the SUBCONTRACTOR under CAPCO's financial monitoring program, <br />CAPCO will review those records and provide the SUBCONTRACTOR a written <br />summary of the findings of that review. CAPCO will also allow the SUBCONTRACTOR <br />reasonable time to respond to any findings of noncompliance or other problems <br />identified by the records review. <br /> <br />(c). SUBCONTRACTOR Close Out Report <br /> <br />No later than thirty (30) days following the termination of this Contract, SUBCONTRACTOR <br />must submit to CAPCO a final "Financial Status Report," which indicates that the report is <br />the "Final Report." If all expenditures have been completed before the end of the Contract, <br />SUBCONTRACTOR shall submit the final "Financial Status Report" with the final "Request <br />for Reimbursement." <br /> <br />(d). Independent Contractor <br /> <br />SUBCONTRACTOR is not an employee or agent of CAPCO, but provides goods and <br />performs services under this Contract solely as an independent contractor. <br /> <br />Article 4: <br /> <br />Reporting Requirements <br /> <br />(a). The SUBCONTRACTOR shall prepare and submit to CAPCO progress reports as designated <br />in Attachment C of this Contract. These reports concern the performance under this <br />Contract documenting program accomplishments and units of work performed under <br />Attachment B of this Contract. These progress reports, to be submitted by the <br />SUBCONTRACTOR, shall include: <br /> <br />1. Form PT-S1 which documents the progress and completion of tasks <br />2. Form PT-R1, the Waste ~iversion Tracking Form <br />3. Forms R1A, R1B, R1D or R1E (as applicable to the project or as requested by CAPCO) <br /> <br />Payments (reimbursements) required under this Contract will be withheld by CAP CO until <br />such time as any past due progress reports are received. <br /> <br />(b). The SUBCONTRACTOR progress reports required under Subsection A of Article 4 contain <br />descriptions of activities and costs for CAPCO to ensure that the provisions of this Contract <br />are being complied with. In particular, any legal research and related legal activities shall <br />be clearly detailed in the quarterly progress reports in order to assure CAPCO that the <br />activities are not prohibited under Attachment 0 Article 2 of this Contract (relating to <br />Supplemental Funding Standards).
