<br />Dog 100 lbs and over $125.00 <br />The above procedures include anesthesia and all medications <br />required. <br />Laboratory Costs: <br />Occult heartworm, Lyme's, and erlichia $35.00 <br />Occult heartworm tests $23.75 <br />Fecal $14.75 <br />Feline Leukemia and Feline Aids Combination $32.75 <br />Office call/examination $23.50 <br />After hours Office call/examination $56.00 <br />Euthanasia $10.00/cc <br />Leaving animal overnight at Veterinarian's office $20.00/day <br />All other services $60/hour <br /> <br />Any other exams, diagnostics, medications and other surgical procedures offered will be <br />charged at regular rates (including emergency/after hours rates, if applicable) in effect at <br />the time and will only be performed with the prior approval of the City's representative. <br />The Veterinarian will review its pricing structure at the end of each twelve month period <br />with the City's representative in order to determine if changes in the fee structure are <br />required due to changes in cost to the Veterinarian. The City will not unreasonably <br />withhold its approval of changes to the fee schedule. <br />B. The Veterinarian will submit monthly InVOICeS describing the serVices <br />rendered, by date and by either procedure or activity/time spent, as applicable, and <br />amounts owed. <br /> <br />e. The fees to be remitted by the Veterinarian to the City under Section 2.E of this <br />agreement, and the compensation provided by the City to the Veterinarian under Section <br />3.A of this agreement relate to the settlement of certain claims between the parties arising <br />under a separate agreement. Those claims were unrelated to the rendition of any <br />professional services by the Veterinarian. Until the total of 1) the low cost <br />spay/neuter/rabies voucher amounts and excess fees remitted by the Veterinarian to the <br />City under Section 2.E. of this agreement, 2) the fees charged by the Veterinarian for <br />services under this agreement, and 3) any other payments made by the Veterinarian to the <br />City under that separate agreement reaches a level of $39,000, the fees remitted to the <br /> <br />Jorgensen Veterinary K.doc <br /> <br />3 <br />
