ATTACHMENT D Wire Transfer Services Security Procedure Agreement New = Replacement Agreement This agreement is made this day of by and between ("you" or "your") and (the "Bank"). By signing. you acknowledge receiving, reading and agreeing to be bound by these terms and conditions and those contained in the Bank's Wire Transfer Customer Setup and Change Form and Wire Transfer Services Agreement brochure. You understand that in any instructions to transfer funds by wire from accounts you maintain at the Bank ("Orders"), the Bank may rely solely (i) on the account number of the person leneficiary") who is to receive the wire transfer rather than the name of that person, and (ii) if provided by you, on the identification number any other financial institution through or to which the funds are to be transferred, rather than the name of the financial institution. .-ndan:I Security Procedures . : The Bank's standard security procedures consist of confirming that the personal identification number ("PIN'1 that accompanies an Order corresponds with a valid PIN assigned to you on voice-initiated and terminal-initiated Orders, and in addition, on terminal-initiated Orders only, SecurlD or other system security features offered by the Bank. A PIN is required for all voice and terminal-initiated transfers. . Additional ~ Procedure Options " no box is checked, you agree that you have not selected any of these additional security procedure options. a. 0 Telephone Verification - Voice Initiated Orders Only: If checked, the Bank will make a reasonable attempt to telephone a person(s) designated by you on the Bank's most current Wire Transfer Customer Setup and Change Form, to verify that a voice-initiated, non-repetitive Order is authorized if it exceeds $ . (If no amount is designated, $500,000 will be used). If the Bank is unable to complete the call the Order will not be processed. The Bank will not telephone to verify a Repetitive Order. A "Repetitive Order" is an Order to the Bank to pay a specified amount of money to a previously designated beneficiary at a previously designated beneficiary's financial institution. b. 0 Other: If checked, you have requested the security procedure described in your signed attachment to this Agreement. The Bank will signify its acceptance only by signing this Agreement and your attachment. Customer Approval (Signatures as required by certificate of authority or signature card portion of your account applicatIon). By signing, you acknowledge and agree that (i) you have had an opportunity to receive and review a description of the Bank's standard security procedures and each of the additional security procedure options described above, and (ii) you are bound by any Order, whether or not authorized, issued in your name and accepted by the Bank in compliance with this Agreement. By choosing to initiate your Orders by ice you are electing not to take advantage of the additional security procedure options available for terminally-initiated orders. Larry D.
