We are <br />very pleased that the City is interested in participating in the program. In reviewing the completed <br />documents, there are some items that I wanted to discuss with you. Your staff notified me that you were on <br />vacation and requested that I return these to your attention. <br /> <br />[t appears that you had signed the Participation Agreement and also had completed the Certificate of <br />Incumbency section as well. I apologize that this document can be somewhat vague. The signature for the <br />Certificate of Incumbency should actually be another individual employed with the City attesting to the first <br />signature. We respectfully request that the Mayor execute the Participant Agreement and the Resolution and <br />either the City Secretary or you attest to the Mayor's signature on each document. I have enclosed new <br />documents and have indicated where each individual will need to sign. I sincerely apologize for any <br />confusion and for the need to revise this paperwork. We will set up your account expeditiously upon receipt <br />of these documents. <br /> <br />Please find enclosed a self-addressed Federal Express envelope for your use to return the documents to the <br />Comptroller's Office at our expense.
