Applications will only be accepted when all required documentation is completed and submitted. You MAY qualify if you: -Are the owner-occupant of the home to be assisted; or 4aintain a ninety-nine.(99) year leasehold interest on the property; and -Have no restrictions or encumbrances that would unduly restrict the good and marketable nature of the ownership interest and -Are not delinquent on property taxes; and -Have a household annual income of less than: --------- w .20" 1 A/ r. 0 M F I I M IT R AV FAM11 VJq17F -------- Application forms will be available and accepted in the City Planning Department located on the 2N floor of the Municipal Building at 630 E, Hopkins, San Marcos, . You MW provide copies of the following documents in order for your application to be considered.' -Determination of Disability/Handicap (if applicable) -Proof of ownership of your home -Proof of occupowy (utility bills/stubs, voter registration, drivers license) -Proof of paid property, taxes (tax receipts) -Proof of income for all occupants of the dwelling over the age of 17 (the most recent past 6 months of check stubs, bank statements, andlor current documentation from government agencies) Applicant will be required to give permission for the verification of all information received.
