<br /> PERFORMING AGENCY will submit water samples to RECEIVING AGENCY for fluoride <br /> level analysis once a month or with less frequency as may be specified in writing by RECEIVING <br /> AGENCY. <br /> PERFORMING AGENCY will assure RECEMNG AGENCY of the continuation of fluoridation <br /> beyond the contract term by continuing to submit monthly water samples to RECEIVING <br /> AGENCY for fluoride level analysis. <br /> PERFORMING AGENCY will provide written justification to RECEIVING AGENCY if the <br /> project has not commenced within thirty (30) days after the execution of the contract. If <br /> PERFORMING AGENCY's project has not commenced within sixty (60) days after execution <br /> of the contract, RECEIVING AGENCY may cancel this contract for noncompliance according <br /> to the TERMINATION Article of the General Provisions. <br /> PERFORMING AGENCY will comply with Part A, Title XIX, Public Health Service (PHS) Act, <br /> as amended, 42 USC § 300w, et seq. (Preventive Health and Health Services Block Grant). <br /> PERFORMING AGENCY will provide services to an estimated 36.000 persons in or benefiting <br /> the county(ies)/area defined as: Hays. <br /> SECTION II. SPECIAL PROVISIONS: <br /> General PrQvisions, SCOPE OF WORK, second paragraph, is replaced with the following: <br /> Satisfactory performance of this contract will be measured in part by: <br /> (1) adherence to the contract; <br /> (2) results of CPA or State Auditor reports; <br /> (3) timeliness, completeness, and accuracy of required reports; and <br /> (4) timely completion and implementation of the fluoridation system. <br /> General Provisions, STANDARDS FOR FIN AN CIAL AND PROGRAMMATIC <br /> MANAGEMENT Article, Item C, and last two paragraphs, are not applicable to this Attachment. <br /> General Provisions, ALLOW ABLE COSTS AND AUDIT REQUIREMENTS, is revised to <br /> include the following: <br /> Funds awarded under this Attachment that are not utilized for a specific fluoridation cost identified <br /> in PERFORMING AGENCY's drawings and equipment component listing may be used to offset <br /> other allowable fluoridation-related costs with RECEMNG AGENCY's prior written approval. <br /> General Provisions, TERMS AND CONDITIONS OF PAYMENT Article, second paragraph, <br /> is replaced with the following: <br /> ATTACHMENT - Page 2 <br />
