<br /> SECTION lli. BUDGET is amended to add the following: <br /> PERFORMING AGENCY agrees to: <br /> (a) submit a separate State of Texas Purchase Voucher for the discretionary funding <br /> initiatives, Le., the peer counseling program and lactation consultants; <br /> (b) submit all forms required by RECEIVING AGENCY, Bureau of Clinical and <br /> Nutritional Services; and <br /> (c) retain appropriate documentation to support expenditures. <br /> ATTACHMENT - Page 2 <br />
