<br /> . <br /> 146 <br /> CITY OF SAN MARCOS HUMAN SERVICES <br /> PROVIDER CONTRACT <br /> STATE OF TEXAS <br /> KNOW ALL PERSONS BY THESE PRESENTS: <br /> ~ --- <br /> I COUNTY OF HAYS <br /> SECTION 1, Parties to Contract: This contract is entered into between the City of San <br /> <br /> Marcos ("City") and San Marcos Youth Services a provider of human services ("PHS"). The parties <br /> agree to be bound by the mutual obligations, perfonnances, duties and accomplishments in this <br /> Agreement. <br /> SECTION 2. Contract Period: This contract shall be in effect from October 1, 1995 <br /> <br /> .¡ <br /> through September 30, 1996. <br /> SECTION 3. Obligation of the City and PHS: City agrees to provide PHS $30,000.00 in <br /> <br /> return for its faithful and satisfactory perfonuance of the activities proposed in its human services <br /> funding application which is attached hereto as Appendix A. <br /> SECTION 4. Reports: PHS agrees to provide quarterly reports as required by the City <br /> <br /> relating to its perforn1ance under the tenus of its human services funding application. The fonnat and <br /> content of these reports will be detenuined by the City's Human Services Advisory Board and the <br /> City. <br /> SECTION 5. Periodic Pavments: Funds to be paid by the City as provided in Section 3 <br /> <br /> above shall be released to PHS in approximately equal quarterly payments begil1Iling ,vith October <br /> 1, 1995, and continuing with the quarters beginning on January 1, 1996, April 1, 1996 and July 1, <br /> 1996. <br />
