<br /> 136 <br /> - <br /> CITY OF SAN MARCOS HUMAN SERVICES <br /> PROVIDER CONTRACT <br /> STATE OF TEXAS <br /> KNOW ALL PERSONS BY THESE PRESENTS: <br /> COUNTY OF HA YS <br /> SECTION 1. Parties to Contract: This contract is entered into between the City of San <br /> Marcos ("City") and Allenwood Senior Citizens a provider of human services ("PHS"). The parties <br /> agree to be bound by the mutual obligations, performances, duties and accomplishments in tbis <br /> Agreement. <br /> SECTION 2. Contract Period: This contract shall be in effect from October 1, 1995 <br /> . <br /> through September 30, 1996, <br /> SECTION 3. Obligation of the City and PHS: City agrees to provide PHS $1,000.00 in <br /> return for its faithful and satisfactory performance of the activities proposed in its human services <br /> funding application which is attached hereto as Appendix A, <br /> SECTION 4, Reports: PHS agrees to provide quarterly reports as required by the City <br /> relating to its performance under the terms of its human services funding application. The format and <br /> content of these reports will be determined by the City's Human Services Advisory Board and the <br /> City. <br /> SECTION 5. Periodic Payments: Funds to be paid by the City as provided in Section 3 <br /> above shall be released to PHS in approximately equal quarterly payments beginning with October <br /> 1, 1995, and continuing with the quarters beginning on January 1, 1996, April 1, 1996 and July 1, <br /> 1996, <br />
