<br /> I ' <br /> 112 <br /> I <br /> CITY OF SAN MARCOS HUMAN SERVICES <br /> PROVIDER CONTRACT <br /> STATE OF TEXAS <br /> KNOW ALL PERSONS BY THESE PRESENTS: <br /> COUNTY OF HAYS <br /> SECTION 1. Parties to Contract: This ~contract is entered into <br /> between the City of San Marcos ("City") and Retired Senior Volunteer <br /> Proqram. a provider of human services II (PHS"). The parties agree to <br /> be bound by the mutual obligations, performances, duties and <br /> accomplishments hereinafter provided. <br /> SECTION 2. Contract Period: This contract s~all be in effect <br /> from October 1, 1994 through September 30, 1995. <br /> SECTION 3. Obliqation of The City And PHS: city agrees to <br /> provide PHS $1,000 in return for its faithful and satisfactory <br /> performance of the activities proposed in its human services funding <br /> application which is attached hereto as Appendix A. <br /> SECTION 4. Reports: PHS agrees to provide quarterly reports <br /> as required by the city relating to its performance under the terms <br /> of its human services funding application. The format and content <br /> of these reports will be determined by the city's Human Services <br /> Advisory Board and the city. <br /> SECTION 5. Periodic Payments: Fund$ to be paid by the City <br /> as provided in Section 3 above shall be released to PHS in <br /> approximately equal quarterly payments beginning with October 1, <br /> -1- <br /> I <br /> , I <br /> I <br />
