C3000606 <br /> ATTACHMENT NO. 01C <br />PERFORMING AGENCY: CITY OF SAN MARCOS <br />RECEIVING AGENCY PROGRAM: BUREAU OF WOMEN, INFANTS, AND CHILDREN'S NUTRITION <br />TERM: October 1, 1992 THROUGH September 30, 1993 <br />It is mutually agreed by and between the contracting parties, to amend the <br />conditions of Document No. C3000606-01-B as written below. All other conditions <br />not hereby amended are to remain in full force and effect. <br />SECTION I. SCOPE OF WORK, first paragraph, is amended to incorporate <br />immunization activities, and reads as follows: <br /> PERFORMING AGENCY will provide supplemental food instruments, nutrition <br /> education, and immunizations as an adjunct to good health care at no cost <br /> to low income pregnant and postpartum women, infants, and children <br /> identified to be at nutritional risk. In meeting this goal, PERFORMING <br /> AGENCY will: <br />SECTION I. SCOPE OF WORK, is amended by adding paragraph F. regarding <br />immunization activities which reads as follows: <br />F. Revie~ the immunization record of WIC program applicants/participants <br /> and provide immunizations as needed in accordance witQ Immunization <br /> Policies provided by RECEIVING AGENCY to PERFORMING AGENCY using staff <br /> funded under this attachment. (See new paragraph relating to reimburse- <br /> ment for immunization activities added to SECTION III. SPECIAL <br /> PROVISIONS, Terms and Conditions of Payment.) <br />The original paragraphs of SECTION III. SPECIAL PROVISIONS, which modify the <br />General Provisions, Terms and Conditions of Payment Article, are replaced in <br />their entirety as shown below. Items 4 and 5 are new provisions relating to <br />reimbursement for expansion of the WIC program to NEW participants and <br />reimbursement for new immunization activities. <br /> The participant caseload quantity will be assigned by RECEIVING AGENCY by <br /> giving written notice to PERFORMING AGENCY and may be subject to change <br /> from time to time upon written notice to PERFORMING AGENCY from RECEIVING <br /> AGENCY. PERFORMING AGENCY assumes liability for all foods costs <br /> resulting from PERFORMING AGENCY exceeding assigned caseload maximum. <br /> The number of individuals served in excess of assigned caseload are not <br /> to be included in the calculation of earned administrative funds as <br /> described below. <br /> RECEIVING AGENCY will reimburse PERFORMING AGENCY for administrative <br /> costs which include participant referral, vendor evaluation and monitor- <br /> ing, nutrition education, general administrative support, start-up costs, <br /> outreach, applicant screening, and food card issuance. <br /> Administrative costs will be reimbursed based on actual costs, but not to <br /> exceed the »maximum reimbursement~ set out below, based upon the sum of <br /> -1- <br />
