<br />133 <br /> <br />CITY OF SAN MARCOS HUMAN SERVICES <br /> <br />PROVIDER CONTRACT <br /> <br />STATE OF TEXAS <br /> <br />KNOW ALL PERSONS BY THESE PRESENTS: <br /> <br />COUNTY OF HAYS <br /> <br />SECTION 1. Parties to Contract: This Contract is entered into <br /> <br />between the C~ty of San Marcos ("City") and Hays Countv Legal Aid. <br /> <br /> <br />Inc. a provider of human services "(PHS"). The parties agree to be <br /> <br />bound <br /> <br />by <br /> <br />the <br /> <br />mutual <br /> <br />obligations, <br /> <br />performances, <br /> <br />duties <br /> <br />and <br /> <br />accomplishments hereinafter provided. <br /> <br />SECTION 2. Contract Period: This contract shall be in effect <br /> <br />from October I, 1993 through September 30, 1994. <br /> <br />SECTION 3. <br /> <br />Obligation of The City And PHS: <br /> <br />City agrees to <br /> <br />provide PHS $10.000.00 in return for its faithful and satisfactory <br /> <br />performance of the activities proposed in its human services funding <br /> <br />application which is attached hereto as Appendix A. <br /> <br />SECTION 4. <br /> <br />I <br />Reports: PHS agrees to provide quarterly reports <br /> <br />as required by the City relating to its performance under the terms <br /> <br />of its human services funding application. <br /> <br />The format and content <br /> <br />of these reports will be determined by the City I S Human Services <br /> <br />Advisory Board and the City. <br /> <br />SECTION 5. <br /> <br />Periodic Payments: Funds to be paid by the City <br /> <br />as provided in Section 3 above shall be released to PHS in <br /> <br />approximately equal quarterly payments beginning with October I, <br /> <br />-1- <br />
