<br /> If'll <br /> QIV SYSTEM OPERATIONAL SUPPORT (QIVSOS) <br /> AGREEMENT FOR SERVICES <br /> SPECIFICATION SUPPLEMENT <br /> QIV System Operational Support, (QIVSOS) provides acœss to a toll-free Customer Support Number and onsite <br /> system support as required, (Monday tbru Friday/8:00a.m. - 5:00p.m. and prescheduled). This service includes <br /> additional equipment and software installation, performance tuning, PTF application, operator training, product <br /> training, problem determination, diagnosis and correction, and other technical services as required. Three alternative <br /> packages (QIVSOS-3, QIVSOS-9, QIVSOS-12) provide comprehensive support and are services priced at rates <br /> substantially discounted from the standard Hourly S.E. Service billing rate. The standard Hourly S.E. Services <br /> billing rate will be used for hours of service provided in excess of those allowed by the QIVSOS package services. <br /> SERVICE DESCRIPTION BILLING RATE INITIAL OPTION <br /> QIVSOS-3: This service provides a total of 24 hours of support $1,200 SPC* <br /> over a three-month period, usually immediately following the <br /> initial system installation, but can be purchased at any time. <br /> QIVSOS-9: This service extends QIVSOS-3 for the balance of a $1,800 SPC* or <br /> year by providing 4 hours of non-accruing support per month for $24O/MONTH <br /> nine months upon the expiration of QIVSOS-3. <br /> QIVSOS-12: This annual service provides 4 hours of non- $2,400 SPC* OJ: $2.400 <br /> accruing support per month for twelve months. $240/MONTH <br /> I TOTAl$, MONTHLY, and/or SPC: $2.400 I <br /> <br /> *SPC = SINGLE PAYMENT CHARGE <br /> Tlûs Agreement is incorporated into and is governed by the tenDS and conditions of the QIV Agreement for <br /> ( Services. <br /> : , <br /> ACCEPTED BY: ACCEPTED BY: <br /> Q.I.
