<br /> DOCUMENT NO. C2000484 <br /> ATTACHMENT NO. 01A <br /> PERFORMING AGENCY: CITY OF SAN MARCOS <br /> RECEIVING AGENCY PROGRAM: BUREAU OF WOMEN. INFANTS. AND CHILDREN'S NUTRITION <br /> TERM: October 1, 1991 THROUGH September 30, 1992 <br /> It is mutually agreed by and between the contracting parties to amend the <br /> conditions of Document No. C2000484-01 as written below. All other conditions <br /> not hereby amended are to remain in full force and effect. <br /> SECTION III. SPECIAL PROVISIONS, ninth paragraph, regarding administrative <br /> costs, is amended to revise the levels of Participants Served per Month and <br /> the Maximum Reimbursement rates as set forth below. The revised rates are <br /> retroactive to October 1, 1991. RECEIVING AGENCY will be responsible for <br /> calculating earned administrative funds under the amended scales, and making <br /> adjustments to previously submitted claims as applicable. <br /> Administrative costs will be reimbursed based on actual costs, but not to <br /> exceed the "maximum reimbursement" set out below, based upon the sum af the <br /> participants who actually receive food instruments each month plus infants who <br /> do not receive any foad instruments whose breastfeeding mothers were <br /> participants, to the extent that the total so derived does not exceed the <br /> PERFORMING AGENCY'S total assigned caseload within any given month. Surplus <br /> funds (the amount by which maximum reimbursements exceed actual cost) can be <br /> accumulated and carried forward within the attachment term. The time period <br /> during which surpluses may be recovered will be determined by the RECEIVING <br /> AGENCY. <br /> RETROACTIVE TO OCTOBER 1, 1991 <br /> PARTICIPANTS SERVED PER MONTH MAXIMUM REIMBURSEMENT <br /> First 500 participants served $ 9.60 per participant <br /> Next 1000 participants served $ 7.95 per participant <br /> Next 8500 participants served $ 5.50 per participant <br /> All additional participants served $ 4.65 per participant <br /> -1- <br />
