DOCUMENT NO. C2000484 ATTACHMENT NO. 01A PERFORMING AGENCY: CITY OF SAN MARCOS RECEIVING AGENCY PROGRAM: BUREAU OF WOMEN. INFANTS. AND CHILDREN'S NUTRITION TERM: October 1, 1991 THROUGH September 30, 1992 It is mutually agreed by and between the contracting parties to amend the conditions of Document No. C2000484-01 as written below. All other conditions not hereby amended are to remain in full force and effect. SECTION III. SPECIAL PROVISIONS, ninth paragraph, regarding administrative costs, is amended to revise the levels of Participants Served per Month and the Maximum Reimbursement rates as set forth below. The revised rates are retroactive to October 1, 1991. RECEIVING AGENCY will be responsible for calculating earned administrative funds under the amended scales, and making adjustments to previously submitted claims as applicable. Administrative costs will be reimbursed based on actual costs, but not to exceed the "maximum reimbursement" set out below, based upon the sum af the participants who actually receive food instruments each month plus infants who do not receive any foad instruments whose breastfeeding mothers were participants, to the extent that the total so derived does not exceed the PERFORMING AGENCY'S total assigned caseload within any given month. Surplus funds (the amount by which maximum reimbursements exceed actual cost) can be accumulated and carried forward within the attachment term. The time period during which surpluses may be recovered will be determined by the RECEIVING AGENCY.
