<br />TEXAS STATE LIBRARY, LSCA TITLE II APPLICATION PAGE 4 <br /> ~ <br />PART I <br />Block 1. The applicant will be the name of your library, and the contact person <br /> should be the individual with whom the State Library will work <br /> throughout the project. <br />Block 2. Provide the estimated dates for bid advertisement, contract award, <br /> and construction completion. <br />Block 3. List the amount of proposed funding by source. The total proposed <br /> funding amount should be the same as the architectural estimate <br /> certified in the preapplication. The total proposed funding amount <br /> should be the same as the total given in Part lit, No.6. <br />Block 4. The Federal Employer Identification Number is the number assigned by <br /> the Internal Revenue Service. <br />Block 6. A copy of the preapplication should have been submitted to your <br /> regional council or planning commission requesting its review. An <br /> identifier number was assigned by the council or planning commission. <br />Block 7. The population served is the population service area assigned by the <br /> State Library under the Rules for the Library Systems Act. <br />Block 8. A response must be given for both questions. <br /> <br />Block 9. Leave blank. <br /> <br />Block 10. Signature of the city manager, mayor, or county judge who is <br /> empowered to enter into agreements binding upon the city or county <br /> government. <br />PART II <br />Questions are self-explanatory. <br />
