<br />CITY OF SAN MARCOS HUMAN SERVICES <br />PROVIDER CONTRACT <br /> <br />STATE OF TEXAS <br /> <br />KNOW ALL PERSONS BY THESE PRESENTS: <br /> <br />COUNTY OF HAYS <br /> <br />SECTION 1. Parties to Contract: This Contract is entered <br /> <br />into between the City of San Marcos ("City") and San Marcos Food <br /> <br />Bank.. provider of human services "(PHS"). The parties agree to <br /> <br />be bound by the mutual obligations, performances, duties and <br /> <br />accomplishments hereinafter provided. <br /> <br />SECTION 2. Contract Period: This contract shall be in <br /> <br />effect from October 1, 1991 through September 30, 1992. <br /> <br />SECTION 3. Obligation of The City And PHS: City agrees to <br /> <br />provide $9,508.00 to PHS in return for its faithful and <br /> <br />satisfactory performance of the activities proposed in its human <br /> <br />services funding application which is attached hereto as <br /> <br />Appendix A. <br /> <br />SECTION 4. Reports: PHS agrees to provide quarterly <br /> <br />reports as required by the City relating to its performance <br /> <br />under the terms of its human services funding application. The <br /> <br />format and content of these reports will be determined by the <br /> <br />City I S Human Services Advisory Board and the City. An annual <br /> <br />audit conducted by an independent auditor or by an agency <br /> <br />finance committee shall be provided to the City on or before <br /> <br />1 <br />
