SPECIAL PROVISIONS <br /> 5.1 TERMS OF THE CONTRACT: Terms of the maintenance contract <br /> shall be for one year from time of award or contract signing <br /> with option for two one year extensions. <br /> 5.2 PRICING: Prices for the first year and the two option years <br /> shall be provided. <br /> 5.3 TERMS OF PAYMENT: Payment of contract will be made in 12 <br /> equal installments per year upon receipt of monthly invoice <br /> from vendor. <br /> 5.4 NOTICE OF TERMINATION: Contract shall provide for a 30 day <br /> "Notice of Termination". <br /> 5.5 QUESTIONNAIRE: Offeror must complete the "Questionnaire". <br /> Failure to respond to all questions could result in a <br /> "Non-Responsive" evaluation. <br /> <br /> 5.6 STANDARD MAINTENANCE CONTRACT: Offeror must provide a copy of <br /> your firms standard Maintenance Contract for the City's <br /> evaluation. <br /> 5.7 FINAL MAINTENANCE CONTRACT: Successful offeror's proposal <br /> will be included in the final maintenance contract. <br /> 5.8 DOCUMENTATION REQUIREMENTS: Offeror must provide written <br /> documentation identifying service and equipment warranties, <br /> 5.9 MANDATORY PRE-BID CONFERENCE: A mandatory pre-bid conference <br /> will be held on (DAY), (DATE) at (TIME) (LOCATION). <br /> 5.10 FAILURE TO MEET SPECIAL PROVISIONS: Failure by offeror to <br /> meet any of the above "Special Provisions" could result in a <br /> "Non-Responsive" evaluation. <br /> <br /> 5.11 EXCEPTIONS TO SECTION III. <br /> 5.11.1 Paragraph 10 Award of Contract <br /> 5.11. 2 Paragraph 22 Payments <br /> 14 <br />
