<br /> 128 <br /> CITY OF SAN MARCOS HUMAN SERVICES ~~ <br /> PROVIDER CONTRACT <br /> STATE OF TEXAS <br /> KNOW ALL PERSONS BY THESE PRESENTS: <br /> COUNTY OF HAYS <br /> SECTION 1. Parties To Contract: This Contract is entered <br /> into between the City of San Marcos ("City") and Hays County <br /> Legal Aid Program , a provider of human <br /> services ("PHS"). The parties agree to be hound by the mutual <br /> obligations, performances, duties and accomplishments hereinafter <br /> provided. <br /> SECTION 2. Contract Period: This contract shall be in effect <br /> from October 1, 1989, through September 30, 1990. <br /> SECTION 3. Obliqations Of The City And PHS: City agrees to <br /> provide $ 29,216.00 to PHS in return for its faithful and satisfactory <br /> performance of the activities proposed in its human services funding <br /> apí?lication which is attached hereto as Appendix A. <br /> SECTION 4. Reports: PHS agrees to provide quarterly reports <br /> 3.S required by the City relating to its performance under the terms of <br /> its human services funding application. The format and content of <br /> these reports will be determined by the City's Human Services Advisory <br /> 30ard and the City. An annual audit conducted hyan indepeodent <br /> 3uditor or by an agency finance committee shall be provided to the City <br /> on or before December 31, 1990. The audit shall follow Clenerally <br /> :3ccepted auditing standards. <br />
