<br /> 130 <br /> CITY OF SAN MARCOS HUMAN SERVICES <br /> PROVIDER CONTRACT <br /> STATE OF TEXAS <br /> KNOW ALL PERSONS BY THESE PRESENTS: <br /> COUNTY OF HAYS <br /> SECTION 1. Parties To Contract: This Contract is en te red <br /> into between the City of San Marcos ("City") and Greater <br /> San Marcos Youth Council , a provider of human <br /> services ("PHS"). The parties agree to be bound by the mutual <br /> obligations, performances, duties and accomplishments hereinafter <br /> pr-ovided. <br /> SECTION 2. Contract Period: This contract shall be in effect <br /> from October 1, 1989, through September 30, 1990. <br /> SECTION 3. Obliqations Of The City And PHS: City agrees to <br /> pr-ovide $ 7,311.00 to PHS in return for its faithful and satisfactory <br /> rer-formance of the activities proposed in its human services funding <br /> apf?lication which is attached hereto as Appendix A. <br /> SECTION 4. Reports: PHS agrees to provide quarter-Iy reports <br /> as r-equired by the City relating to its performance under the terms of <br /> its human services funding application. The for-mat and content of <br /> these r-epor-ts will be deter-mined by the City's Human Ser-vices Advisory <br /> 30ar-d and the City. An annual audit conducted by an independent <br /> auditor or by an agency finance committee shall be provided to the City <br /> on or- before Decembe r 31, 1990. The audit shall follow Qenerally <br /> accepted auditing standards. <br />
