GM-12 <br />(herein called the Contract) <br /> <br />with <br /> <br />CITY OF SAN MARCOS <br />(herein called the Enrolling Unit) <br /> <br />The Central Texas Health Plan (herein called Health Plan) hereby agrees <br />with the Enrolling Unit to provide the health services set forth herein to <br />Covered Persons, subject to the excltlsions, limitatir::.ns, conditiol'"ls and <br />othet~ terms of this C.:mtt'ðct. <br /> <br />This Contract is made in consideration of the Enrolling Unit's application <br />and Enrollee's application, if any, and payment of the required Contract <br />Charges as specified herein.
