<br />DEFINITIONS <br /> <br />Certain words and phrases used in this Plan Document are listed below, along <br />with the definition or explanation of the manner in which the term is used for <br />the purposes of this Plan. <br /> <br />Masculine pronouns used in this Plan Document shall include masculine or <br />feminine gender unless the context indicates otherwise. <br /> <br />Wherever any words are used herein the singular or plural, they shall be <br />construed as though they were in the plural or singular, as the case may be, <br />in all cases where they would so apply. <br /> <br />ACTIVELY AT WORK <br /> <br />The term "Actively at Work" means the active expenditure of time and energy in <br />the service of the Employer. A participant shall be deemed actively at work <br />on each day of a regular paid vacation, or on a regular non-working day on <br />which he is not totally disabled, provided he was actively at work on the last <br />preceding regular working day. <br /> <br />AMBULATORY SURGICAL CENTER <br /> <br />The term "Ambulatory Surgical Center" means an institution or facility, either <br />free standing or as part of a hospital with permanent facilities, equipped and <br />operated for the primary purpose of performing surgical procedures and to <br />which a patient is admitted and discharged within a twenty-four (24) hour <br />period. An office maintained by a physician for the practice of medicine or <br />dentistry, or for the primary purpose of performing terminations of pregnancy, <br />shall not be considered to be an ambulatory surgical center. <br /> <br />AMENDl-tENT <br /> <br />The term "Amendment" means a formal document that changes the provision:;J of <br />the Plan Document, duly signed by the authorized person or persons as <br />designated by the Plan Administrator. <br /> <br />APPROPRIATE. APPROPRIATENESS <br /> <br />The term "Appropriate" or "Appropriateness" refers to the classification of a <br />medical service as customary and usual for the treatment of any given medical <br />condition. Such services must be commonly recognized by the medical <br />profession as an accepted standard for that type and level of care. <br /> <br />BENEFIT PERCENTAGE <br /> <br />The term "Benefit Percentage" means that portion of eligible expenses to be <br />paid by the Plan in accordance with the coverage provisions as stated in the <br />Plan. <br /> <br />7 <br />
