113 STATE OF TEXAS CITY OF SAN MARCOS HUMAN SERVICES PROVIDER CONTRACT COUNTY OF HAYS KNOW ALL PERSONS BY THESE PRESENTS: SECTION 1. Parties To Contract: This Contract is made and Marcos (flCity") and provider of human en tered into by and between the Ci ty of San a services (IIPHSfI). The parties hereto agree and by executing this con tract are bound to the mutual obligations, performances, duties and accomplishments hereinafter provided. SECTION 2. Contract Period: This contract shall he in effect from October 1,1988, through September 30, 1989. SECTION 3. Obligations Of The City And PHS: City ag rees to provide $ to PHS in return for its faithful and satisfactory performance of the activities proposed in its human services fund ing application which is attached hereto as Appendix A and incorporated herein in its entirety as though recited verbatim in this agreement. SECTION 4. Reports: PHS agrees to provide certain reports as required by the City relating to its performance under the terms of its human services funding application. Such reports will be made quarterly, the format and content of which will be determined by the City's Human Services Advisory Board and the City. An annual audit conducted by an independent auditor or by an agency finance committee shall be provided to the City on or be fore December 31 , 1989. The audit shall follow generally accepted auditing standards. SECTION 5. Periodic Payments: Funds to be paid by the City as provided in Section 3 above shall be released to PH Sin approximately equal quarterly payments beginning with October 1, 1988, and continuing with the quarters beginning on January 1, 1989, April 1, 1989 and eJuly 1, 1989. SECTION 6. Monitoring Of Performance: PHS agrees to allow the Ci ty , either directly or through its Human Services Advisory Board, to inspect, at City's request, and with advance notice to PHS, all pertinent records, files, in forma tion or other written material
