<br />RECEIVING AGENCY agrees to break out separately on the face of the claim for <br />reimbursement (State of Texas Purchase Voucher, TDH form #AG- 37) the costs <br />associated with nutrition education versus other administrative costs. <br /> <br />ADVANCE PAYMENTS <br />General Provisions, <br />Attachment. <br /> <br />Advance Payments Article is <br /> <br />not applicable <br /> <br />to <br /> <br />this <br /> <br />PROGRAM INCOME <br />General Provisions, Program Income Article, paragraph one is not applicable to <br />this Attachment. <br /> <br />FINANCIAL REPORTS <br />Annual/Final: A final report, Financial Status Report, State Supplemental Form <br />269 (TDH Form GC-3) shall be submitted not later than December 30, 1989. An <br />amended Form 269a shall be submitted if the amount of expenditures reported in <br />the last quarter changed. If necessary, a State of Texas Purchase Voucher will <br />be submitted if all costs have not been recovered or a refund will be made of <br />excess monies if costs incurred were less than funds received. <br /> <br />REPORTS & INSPECTIONS <br />PERFORMING AGENCY shall submit monthly performance data which includes: Report <br />of Program Operations, AC 80-1, Report of Card Sequence Used, AC 80-3, and Cards <br />Voided, AC 80-4. Lost/Stolen Report, AC 78-10, shall be submitted as necessary. <br />On an annual basis, a Racial/Ethnic Group Participation Form, FNS-l9l, for the <br />reporting month of September shall be submitted by the seventh working day of <br />October. <br /> <br />TERMINATION <br />In addition to the Termination Article in the General Provisions, guidelines in <br />the State WIC Policy and Procedure Manual require that RECEIVING AGENCY shall <br />provide persons aggrieved by any of the PERFORMING AGENCY's determinations a <br />prompt opportunity for a fair hearing. <br /> <br />Unilateral termination of this contract by RECEIVING AGENCY for cause shall be <br />sub j ec t to review on appeal by PERFORMING AGENCY in accordance wi th State WIC <br />Policy and Procedure Manual. <br /> <br />SECTION IV. <br /> <br />BUDGET <br /> <br />All categories of costs billed to the RECEIVING AGENCY and allocations of such <br />costs must be in accordance with the RECEIVING AGENCY approved "Plan to Allocate <br />Direct Costs," (PADC). A listing of the categories contained in the negotiated <br />PADC are as shown in Exhibit A of this Attachment. <br /> <br />ATTACHMENT <br />Page 4 <br />
