<br />NOTICE OF VIOLATION <br /> <br />Normal violations defined as follows: <br />1. Failure to posses a valid permit <br />a. Expired <br />b. Unpaid <br />c. Not issued <br />Improper construction or maintenance of pretreatment facilities <br />a. Missing or broken inverts <br />b. Non-existent or improperly located sample ports <br />c. Grossly overloaded traps <br />Exceeding ordinance limits <br />a. Prohibited or limited constituents as stated in <br />Industrial Waste Ordinance <br />Conditions that will rsult in a violation as of the type 3a. <br />Include, but not limited to: <br />1. Insufficient supply of a pretreatment <br />(hydroxide, chlorine, visulfite, etc.) <br />Failure of solution mixing equipment <br />Failure of pretreatment chemical metering pumps <br />Failure of pretreatment solenoid control valves <br />Failure of pH meter, or probe, used to control <br />pretreatment <br />6. Operational errors <br />.Other., as identified by the Environment and Engineering <br />Division. <br /> <br />2. <br /> <br />3. <br /> <br />the <br /> <br />b. <br /> <br />chemical <br /> <br />2. <br />3. <br />4. <br />5. <br /> <br />c. <br /> <br />The Notice of Violation will be completely, clearly, and <br />accurately filled out, a specific violation stated, and a specific <br />corrective action required. An appropriate due date (see the <br />following Guidelines) is stated, by which time the discharger must <br />attain compliance. The Notice will be served to a responsible person <br />(owner, manager, supervisor) and all terms of the Notice will be <br />verbally explained to that person, who also signs the Notice. The <br />original Notice is given to the discharger; one carbon copy is <br />retained for the permit file, and a second carbon copy is sent to the <br />Superintendent of Public Works. <br /> <br />-63- <br />
