Cumulative jobs on or before December 31, 2017: 95 <br />Cumulative jobs on or before December 31, 2018: 125 <br />The average annual salary of such permanent full -time employees shall be at least $65,000.00 during <br />each year above. Each such employee shall be also eligible for group health insurance benefits. <br />Section 3.04. Request for Local Employment Incentive. Upland may request a grant <br />payment of $25,000.00 for each year during the term of this Agreement in which Upland meets the <br />employment requirements set forth in section 3.03, subject to the procedures set forth in this <br />Agreement. Upland's request for payment shall be evidenced by delivery of a sworn statement <br />signed by an authorized officer of Upland and in a form acceptable to the City Manager of the City <br />that Upland verifying, among other things, the following information: <br />(a) the number of permanent full -time employees employed in the City of San <br />Marcos during the reporting period and cumulatively since the Effective Date; <br />(b) the combined average annual salary of such persons employed during the <br />reporting period and cumulatively since the Effective Date; and <br />(c) whether such employees are eligible to receive group health insurance <br />coverage offered by Upland. <br />The sworn statement shall be accompanied by current reports or other official forms acceptable to <br />the City Manager of the City filed with a governmental entity confirming the information contained <br />in the sworn statement. Such forms may include IRS Forms W -2 or 941, or similar reports provided <br />to the Texas Workforce Commission or U.S. Department of Labor. <br />Section 3.05. Time for Submitting Payment Request. Upland's request for payment must <br />be made on or after January 31 of the calendar immediately following the year during which the <br />employees subject to the application were employed.
