Each such employee shall be also eligible for group health insurance benefits. Section 3.04. Request for Local Employment Incentive. Upland may request a grant payment of $25,000.00 for each year during the term of this Agreement in which Upland meets the employment requirements set forth in section 3.03, subject to the procedures set forth in this Agreement. Upland's request for payment shall be evidenced by delivery of a sworn statement signed by an authorized officer of Upland and in a form acceptable to the City Manager of the City that Upland verifying, among other things, the following information: (a) the number of permanent full -time employees employed in the City of San Marcos during the reporting period and cumulatively since the Effective Date; (b) the combined average annual salary of such persons employed during the reporting period and cumulatively since the Effective Date; and (c) whether such employees are eligible to receive group health insurance coverage offered by Upland. The sworn statement shall be accompanied by current reports or other official forms acceptable to the City Manager of the City filed with a governmental entity confirming the information contained in the sworn statement. Such forms may include IRS Forms W -2 or 941, or similar reports provided to the Texas Workforce Commission or U.S. Department of Labor. Section 3.05. Time for Submitting Payment Request. Upland's request for payment must be made on or after January 31 of the calendar immediately following the year during which the employees subject to the application were employed.
