3. This is a cost reimbursement contract. Approved expenses must be paid <br />by the entity prior to reimbursement. <br />4. Signature of the Medical Director is required on proposals of ALL direct <br />patient care providers (licensed EMS providers and registered first <br />responder organizations). Refer to Form E: Medical Director Signature <br />Page. An educational facility or RAC with a proposal involving EMS <br />education courses, including continuing education or system development <br />projects, are not required to submit Form E: Medical Director Signature <br />Page. <br />5. Matching Fund Requirements. <br />a. An individual equipment or supply item with a useful life of more than <br />one year and a cost greater than $1,000 (including shipping costs) will <br />require at least 50% matching funds, except; <br />b. Computers, software, printers, fax machines, stereo equipment, <br />cameras, video recorders/players, medical laboratory equipment <br />(defined as microscopes, oscilloscopes, centrifuges, balances, and <br />incubators) with a useful life of more than one year and a cost greater <br />than $500 (including shipping) will require at least 50% matching <br />funds. <br />Matching funds may come from sources such as local funds, private <br />donations, other state grants, federal grants, or private foundations. <br />6. EMS Certification Training Projects. <br />a. Verification of completed certification program or training course must <br />be submitted within 30 days following the end of the contract period. <br />b. Verification of completed certification programs/training must be <br />submitted before reimbursement can be considered. <br />c. Emergency Care Attendant Training (ECAT) courses may not be <br />funded through FY 10 EMS Local Projects Grant program if they <br />qualify for funding through the DSHS ECAT program. Joint Proposals and Multiple Proposals. <br />The respondent may not submit more than one proposal, except, <br />respondents from an educational facility or RAC for EMS education <br />courses, including continuing education or system development projects. <br />These projects will not prevent respondents from submitting another <br />proposal on their own behalf. <br />The respondent may submit a joint proposal, referred to in this RFP as a <br />multiple entity proposal. DSHS supports the concept of cooperative <br />proposals that clearly demonstrate and document regional projects <br />involving multiple service organizations.
