more than one entity. Projects shall be listed in order of priority. FORM E: <br />Medical Director Signature Page must be included for each licensed <br />provider or registered first responder organization in the proposal. <br />a. One contract will be drawn between DSHS and the respondent <br />representing the group of entities. <br />b. Each entity request in a multiple entity proposal must be <br />individually listed on Form I: Funding Request Budget Page. Selected <br />respondents are responsible for maintaining a record of all costs and <br />activities related to the administration of the project. <br />8. Disaster Services. In the event of a local, state, or federal emergency, <br />including natural, man-made, criminal, terrorist, and/or bioterrorism events, <br />declared as a state disaster by the Governor, or a federal disaster by the <br />appropriate federal official, Contractors may be called upon to assist DSHS <br />in providing services, as appropriate, in the following areas: community <br />evacuation, health and medical assistance; assessment of health and <br />medical needs; health surveillance; medical care personnel; health and <br />medical equipment and supplies; patient evacuation; in-hospital care and <br />hospital facility status; food, drug, and medical device safety; worker health <br />and safety; mental health and substance abuse; public health information; <br />vector control and veterinary services; and victim identification and mortuary <br />services. Disaster services shall be carried out in the manner most <br />responsive to the needs of the emergency, be cost effective, and be least <br />intrusive on the primary services of the Contractor. <br />F. Funding Preferences and Limits <br />No single entity respondent will receive a total award of more than fifty <br />thousand dollars ($50,000). No multiple entity respondents will receive a <br />combined total award of more than eighty-five thousand dollars ($85,000). <br />Funding preferences and limits will be used to determine awards. Funds may <br />be used for EMS personnel certification training, specialty training related to <br />pre-hospital health care management, communication equipment, patient <br />care equipment including the purchase of an ambulance and non-disposable <br />supplies, injury prevention projects, and continuing education programs. <br />DSHS prefers to fund projects which: <br />• clearly demonstrate need; <br />• encourage growth/improvement of regional EMS/trauma system and increase <br />the level of care of the provider; <br />• benefit multiple entities; <br />• consider the needs of the regional EMS/trauma system; <br />• represent the most cost-efficient use of funding; and <br />• involve an ambulance purchase price of less than $100,000. <br />Page 12 RFP# EMS/LPG - 0328.1
