STATEMENT OF WORK: <br />Contractor shall sterilize dogs and cats owned by Texas residents based on the <br />Contractor's approved funding request. <br />Contractor shall comply with all applicable federal and state laws, rules, and regulations <br />including, but not limited to the following: <br />Texas Health and Safety Code, Chapter 828, Dog and Cat Sterilization; <br />htt::!%t1a2.tlc.state.tx.us(statutes;'docs?llSicantei7t?htm(1is.010.00.000828.00.htm <br />Title 25, Texas Administrative Code (TAC), Chapter 169, Subchapter E, <br />§ 169.102, Department of State Health Services (DSHS) Animal Friendly Grants. <br />http•//info} 5()5 state tr us/pIs Duty'readtac$ext.1'acl)age'?51-k&app=--9&p dir-&p r <br />loc°-& tlc>c -& l oc -& T= l & p tac=&ti-25 & . t-=1 &ch 169&.ri-102 <br />Contractor shall perform all activities under this Program Attachment in accordance with <br />a detailed budget as approved by DSHS. Contractor must receive written approval from <br />DSHS before varying from applicable policies, procedures, protocols, and the final <br />approved workable plan, and must update its implementation documentation within forty- <br />eight (48) hours of making approved changes so that staff working on activities under this <br />Program Attachment knows of the change(s). <br />The following documents are incorporated by reference and made a part of this Program <br />Attachment: <br />DSHS' Competitive Request for Proposal (RFP) for Animal Friendly Grants RFP <br />Zoonosis-0324.10 2010, issued February 27, 2009; and <br />Contractor's FY' 10 Animal Friendly Grants RFP Application RFP #: <br />ZOONOSIS-0324.1 2010 and any revisions agreed upon in writing by Contractor <br />and DSHS. <br />DSHS reserves the right, where allowed by legal authority, to redirect funds in the event <br />of financial shortfalls. DSHS will monitor Contractor's expenditures on a quarterly basis. <br />If expenditures are below that projected in Contractor's total Program Attachment <br />amount, as shown in SECTION VII.
